O’NA HealthCare™ is dedicated to providing you and your family with more choices, a variety of treatment options, and excellent client services.
If you decide to cancel you policy we require exactly 30 days notice and that we will only cancel on the 1st or 15 of the next due date after the 30 days.
Also please note that all receipts must be submitted no later than 30 days after the cancellation date or plan end date. It is the members responsibility to validate that we have received your receipts. We will confirm via Fax or email. All provider/hospital claims must be submitted 90 days after the service date that is within your benefit period.
Appeals must be filed 30 days from the decision date following our appeals policy or dispute a provider charge policy.
